Same colony address across chains

Implement deterministic contract address for a colony so that colonies can operate seamless on different chains.

There have been several instances where users have experienced a loss of funds by mistakenly sending funds to a colony address on a chain that colony does not support. This creates a really bad experience for users, even if not a direct result of Colony.

To help combat this, and with Colony adding multi-chain support, we should also endeavor to ensure colonies have the same address across chains.

In implementing multi-chain support with the same address on multiple chains, we should be able to reduce the risk of lost funds in this way while also enhancing functionality.
